Private Practice Agreement

TERMS AND CONDITIONS OF COUNSELLING
I believe an agreement between counsellors and clients to be both desirable and beneficial
before commencing counselling. This document provides the basis of our agreement.


This is what you can expect from me
❖ A confidential, supportive and consistent relationship. To work towards coming to terms
with ~ and managing the difficulties in your life.
❖ Your right to confidentiality will be fully respected in accordance with the Code of
Ethics, and Practice of the British Association for Counselling and Psychotherapy of
which I am a member: Registration Number 632390.and the Code and Ethics of the of the
National Counselling Society of which I am an accredited member Registration Number
NCS14-01196
❖ In order to maintain good practice, I regularly undergo supervision. Please be assured that
your identity will be kept anonymous.
❖ If I have grounds for believing that you are likely to harm either yourself or others, I may
consult with my Supervisor with regard to a possible breach of confidentiality,
particularly in relation to child protection. If this situation were to arise, I would discuss
my intention of such a possible breach of confidentiality with you and would proceed
only with your knowledge and, hopefully, your consent.
❖ It may be considered that counselling is not the best response to your needs. Should this
happen, we will discuss this, but I reserve the right to terminate the counselling session at
any time.


Your part of the agreement – This is your commitment
❖ Payment in advance is appreciated although this can be discussed in the assessment
❖ If you wish to cancel or change an appointment, I would require you to give me 24 hours
notice. If you do not contact me to cancel or change your appointment within this time, I
would expect a contribution from you for the lost time.
❖ To cancel or change an appointment please telephone 07801 450324 leaving your name
and contact number.
❖ If you miss one or more appointments with me, without prior notification, then you may
lose your place for counselling.
❖ You have the right to terminate this agreement at any time.
From both Client and Counsellor
❖ There will be a payment of £45 per session required after our initial assessment, before
either counsellor or client commit to this agreement. (This session will be approximately
50 minutes)
